    Job duties include but are not limited to the following:

    • Provides direct personal care to the residents by performing the following:
      • Assists residents with toileting several times daily and as needed. Provides catheter care as needed. Follows bowel and bladder training programs and keeps incontinent residents clean and dry.
      • Assists residents with feeding and hydration. Prepares and serves nourishments as ordered. Maintains an awareness of residents’ nutritional needs and dining assistance required. Monitors dietary intake. Notifies RN or LPN of any abnormalities in food and fluid intake.
      • Assists residents with bathing.
      • Dresses and/or assists residents with dressing in an appropriate and comfortable manner.
      • Brushes teeth and/or assists residents with brushing teeth, oral hygiene and dental care.
      • Provides proper care to residents to prevent skin breakdown.
      • Performs or assists residents with other personal hygiene requirements, such as combing and brushing hair, cleaning and trimming fingernails and shaving.
      • Checks each assigned resident a minimum of four times per shift. Follows the resident’s Support Plan and provides appropriate care.
      • Answers call lights promptly and provides assistance as requested.
      • Observes residents and notes physical and emotional condition. Reports any changes or unusual findings to the Administrator or the LPN.
      • Performs all required charting in a legible, accurate and timely manner.
      • Uses aseptic techniques in all resident care in order to prevent the spread of disease and infection.
      • Wears protective clothing and devices as required and appropriate.
      • Performs and/or assists with basic nursing procedures such as taking and noting vital signs, collecting specimens and other nursing procedures.
      • Requests appropriate supplies for resident care and follows proper procedures for cost reimbursement.
      • Prepares residents for tests, x-rays, therapy programs and physician’s visits as assigned.
     
    • Provides functional and restorative care, including transfers/transports, ambulates with residents, assist with exercise programs, assist residents with adaptive devices.
     
    • May act as the primary care provider to deliver individualized resident care such as receives/gives reports to next shifts regarding pertinent information and duties to be completed.  Make rounds and observes resident’s conditions,
     
    • Performs various housekeeping duties throughout the Personal Care Facility, including laundry and linen assistance.
    • Spends time visiting with residents and performs personal services such as reading, taking resident s outside, keeping residents informed of current events, arts and crafts, and other recreations activities set forth from the Administration.
    • Prepares trays according to regular or special menus and delivers trays to assigned location/room. Serves meals to residents in main dining room, keeps a clean and sanitized area at all times.
    Qualification
    • Candidate must possess high school diploma or GED.  Previous experience working in a health care or long-term care facility preferred.
