    ACT Team Mental Health Therapist
    CenClear is growing a new program and seeking applicant for several positions. The
    program is Assertive Community Treatment (ACT). ACT is an evidence-based practice
    where staff work together as a team to help improve outcomes for people with severe
    mental illness who are most at-risk of psychiatric crisis, substance use, hospitalization and involvement in the criminal justice system.
    General Description of Duties
    To provide intensive, individualized, and comprehensive delivery of mental health
    services to the most vulnerable consumers in our community in collaboration with other
    members of the treatment team while adhering to ACT regulations/guidelines.
    Location: This position will be in the community with travel to clients homes and to local CenClear offices.
    Hours: 40 hours per week Nontraditional hours
    This position will require travel to multiple offices

    - Master's Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services), or other related field which includes a practicum in a health or human service agency, licensed eligible

    Specific Description of Duties
    · Provides consistent, high quality recovery-oriented and individualized treatment in
    keeping with agency philosophy and best practices.
    · Provides therapy in a professional code of ethic.
    · Promotes a physically and emotionally safe, welcoming therapeutic environment.
    · Works cooperatively with the ACT team to meet the individual needs of clients.
    · With the use of shared decision making with the individual, coordinates referrals and
    exchange of records as needed with appropriate release of information.
    · Provides therapeutic interventions to individuals and families that is culturally respectful to individual strengths, ideas, and choices.
    · Utilizes evidence-based practices and outcome measurements to assess level of care, and growth and change throughout treatment.
    · Develops individualized, recovery-oriented treatment plans with Individuals that
    promotes overall physical and behavioral health well-being.
    · Collaborates with Individuals on goals for discharge and aftercare planning to support
    ongoing growth and change.
    · Maintains up-to-date medical records as specified by state/program regulations.
    · Completes time slips and other documentation as identified within the required time frames.
    · Completes all documentation within the electronic health record (EHR).
    · Schedules appointments based on the needs of consumers and/or authorizations.
    · Submits supporting documentation within 24 hours of the point of service.
    · Maintains ACT training in ACT philosophy, organizational structure and roles,
    standards and practices, management, leadership and EBP embedded in ACT.
    · Additional training in area of expertise.
    · Cross training of team members' functional roles.
    · 20 CEU hours per year focusing on ACT competency standards and practices.
    · Participates in required in-house and appropriate out-of-house trainings.
    · Attend staff meetings.
    · Formal group supervision and formal individual supervision twice per month.
    · Informal supervision occurs during normal course of treatment and collaboration.
    · Completes supervisory services in EHR.
    · Completes other tasks as requested by Project Director/Leadership team.
    · Complies with all memos and procedures upon receipt.
    · Complies with supervisors' request within deadlines given.
