    CenClear is growing a new program and seeking applicant for several positions. The
    program is Assertive Community Treatment (ACT). ACT is an evidence-based practice
    where staff work together as a team to help improve outcomes for people with severe
    mental illness who are most at-risk of psychiatric crisis, substance use, hospitalization and involvement in the criminal justice system.
    General Description of Duties
    Provide intensive, individualized, and comprehensive delivery of health services to the most vulnerable consumers in our community in collaboration with other members of the treatment team while adhering to ACT regulations/guidelines.
    This position will work at CenClear's Mental Health offices within the ACT Team.
    CenClear has a Covid-19 vaccine mandate in effect for all employees by January 31, 2022. Religious and/or medical exemptions will be reviewed.
    · State of Pennsylvania Licensure in good standing, Clinical experience in the care of
    psychiatric patients is desirable.
    · Must have and maintain Act 33, Act 34 and FBI clearances.

    Specific Description of Duties
    · Assess psychiatric medications, monitors ongoing medication changes, and follow-up
    visits, medication syndrome, and periodic consultation with community resources
    - Provides feedback, as needed to prescribing supervisor.
    - Orders and reviews appropriate labs as needed consult with prescribing supervisor
    when necessary for authorizations of medications and refills.
    - Accurately documents in accordance with COMAR regulations in (EMR), Electronic
    Medical Record Compile authorization requests and misc. reports.
    · Maintains up-to-date medical records as specified by state/program regulations.
    · Schedules appointments based on the needs of consumers and/or authorizations.
    · Submits supporting documentation within 24 hours of the point of service.
    · Cross training of team members' functional roles.
    · 20 CEU hours per year focusing on ACT competency standards and practices.
    · Attends training to meet requirements of the licensure and as needed or mandated by
    State of Pennsylvania Board of Nursing and other state/federal/ program regulations.
