    Blended Case Worker at Children’s Aid Society – Clearfield, PA
    This position assists with daily operations of the Adoption/Foster Care Programs, Youth Mentoring Program and provides intensive, strengths-based, in-home services to families
    Job Functions:
    · Provide pre-to-post placement services to prospective adoptive parents.
    · Provide full range of Statewide Adoption and Permanency Network services.
    · Provide Birth Parent counseling
    · Provide intercountry services to prospective adoptive parents to include: assessment, home study preparation, counseling, parent preparation, search work, preparation of court documents, post-placement visits and reports, and attendance at finalization hearings.
    · Participate in Adoption Information Nights.
    · Assess volunteer “fit” for mentoring. Conduct volunteer and child enrollments. Assess and refer families for alternate or additional services as needed.
    · Review all enrollment information and assessments and make recommendations for participation in the program.
    · Provide quality match contact to ensure child safety, match relationship development, positive youth development and volunteer satisfaction. Identify and address real and/or potential problems and barriers.
    · Conduct exit interview with all parties at match closure.
    · Identify and promote re-engagement of volunteers, other capacities or as donors.
    · Assist with all program social outings and facilitate groups as needed.
    · Actively participate in all fundraising activities.
    · Assess individuals and families, using interviewing, discussing, testing, and observing techniques and periodic review of needs and progress.
    · In conjunction with the family, develop and execute service plans to address family needs.
    · Attend interdisciplinary meetings and court proceedings as needed.
    · Counsel, instruct and coach family members on concerns like: communication; child rearing, structure and positive discipline; financial management; home organization, cleanliness and management; medical, dental and mental health needs.
    · In collaboration with the family, provide linkages and consultation, assistance in accessing services such as child care, transportation, housing, mental health, food banks, support groups, treatment facilities, legal aid, etc.
    · Attend staff meetings/trainings as scheduled. Be available to work non-traditional hours on a regular basis and to work emergency hours occasionally, which include, but are not limited to, evening and weekend hours. Participate in the on-call process.

    Job Type: Full-time
    Salary: $28,050.00 - $30,250.00 per year
